Upon making your appointment with Harbor Chiropractic, we will send you an email with a secure link to complete our online intake forms prior to joining us in our office. If there are any issues with completing the online intake forms, we ask that you please print and fill out the following documents to decrease the wait time on your first visit.

The doctor will address your health history and reasons for visiting our office. A thorough examination will be performed, and any necessary x-rays will be obtained. We rarely adjust on the first visit as we like to correlate your health history, current issues, any imaging, and exam findings prior to establishing your proper treatment and plan.
